How do you switch patients from other glucagon-like peptide-1 receptor agonist (GLP-1 RA) medications to Foundayo™ (orforglipron)?

How to switch patients from GLP-1 receptor agonist medications to orforglipron?

The orforglipron US Prescribing Information does not include recommendations on switching from other glucagon-like peptide-1 receptor agonist (GLP-1 RA) medications to orforglipron.1

Eli Lilly and Company cannot provide recommendations beyond the approved label on

  • how to switch a patient from other GLP-1 RA medications to orforglipron
  • dose equivalence between other GLP-1 RA medications and orforglipron, or
  • washout periods between discontinuing another GLP‑1 RA and initiating orforglipron.

The health care practitioner should consider the potential risks and benefits of treatment options and monitor them appropriately.

How were participants switched from injectable semaglutide or tirzepatide to orforglipron in ATTAIN‑MAINTAIN?

This response presents data from the investigational orforglipron capsule formulation (1 mg, 3 mg, 6 mg, 12 mg, 24 mg, and 36 mg) shown as equivalent doses of once-daily orforglipron tablets (0.8 mg, 2.5 mg, 5.5 mg, 9 mg, 14.5 mg, and 17.2 mg) approved in the United States.1-3

ATTAIN-MAINTAIN evaluated a specific switching scenario that may not represent clinical practice scenarios. This study does not allow general conclusions on how to switch from other GLP-1 RA medications to orforglipron.2

The ATTAIN-MAINTAIN study evaluated the use of orforglipron for weight maintenance after an initial treatment with injectable semaglutide or tirzepatide.2

Participants in ATTAIN-MAINTAIN switched from a maximum tolerated dose (MTD) of injectable semaglutide (1.7 mg or 2.4 mg) or tirzepatide (10 mg or 15 mg) to a 9 mg dose of oral orforglipron. After 4 weeks on the 9 mg dose, the orforglipron dose was escalated to 14.5 mg for 4 weeks, and increased after another 4 weeks to reach 17.2 mg or MTD (14.5 mg or 17.2 mg).2

The first orforglipron dose was ideally administered within 14 days of the last dose of either tirzepatide or semaglutide.2

The dose escalation method used in ATTAIN-MAINTAIN was selected to optimize gastrointestinal tolerability based on assessment of different starting doses and escalation intervals used in the phase 1 and 2 studies.2

Additionally, given that the study participants completed 72-week treatment with tirzepatide or semaglutide prior to enrollment, a higher starting dose of orforglipron 9 mg was expected to be reasonably tolerated in this group while avoiding any potential weight regain that might have occurred by starting at a lower starting dose of orforglipron.2

Lilly has not studied the effect on weight after switching to orforglipron doses of 0.8 mg, 2.5 mg, or 5.5 mg; therefore, no data are available.

What Effect Did Switching From Injectable Semaglutide or Tirzepatide to Orforglipron Have on Weight?

In ATTAIN-MAINTAIN, participants who achieved a body weight plateau with tirzepatide in SURMOUNT-5 maintained 74.7% of their body weight reduction after transitioning to orforglipron, representing a weight regain of approximately 5 kg (11 lb) after 52 weeks.2

Participants who achieved a body weight plateau with injectable semaglutide in SURMOUNT-5 maintained 79.3% of body weight reduction after transitioning to orforglipron, representing a weight regain of approximately 1 kg (2 lb) after 52 weeks.2

What Effect Did Switching From Injectable Semaglutide or Tirzepatide to Orforglipron Have on Glycemic Control?

Participants with any type of diabetes were excluded from the SURMOUNT-5 and ATTAIN-MAINTAIN studies. No glycemic control results have been published.2

Lilly has not sponsored a clinical study to evaluate switching from GLP-1 RA medications to orforglipron in patients with type 2 diabetes.

Are there data on switching from other GLP-1 receptor agonists to orforglipron?

Eli Lilly and Company has not evaluated the efficacy and safety of orforglipron following treatment with GLP-1 RA medications other than injectable semaglutide and tirzepatide. No studies have evaluated the use of orforglipron following oral semaglutide.
