How is Foundayo™ (orforglipron) metabolized and eliminated?

Orforglipron is metabolized primarily via hepatic CYP3A4 to several oxidative metabolites. Orforglipron has a half-life of approximately 29 to 49 hours after an oral dose, and a mean systemic clearance of 7.15 L/hour.

Pharmacokinetic Characteristics of Orforglipron Metabolism and Elimination

Orforglipron is a novel small-molecule, non-peptide GLP-1 receptor agonist designed for once-daily oral administration with or without food and water restrictions.1

The pharmacokinetics of orforglipron is similar between healthy subjects and patients with overweight (body mass index ≥27 kg/m2) or obesity.2 

Steady state exposure is achieved after approximately 1 week of once daily administration.2

Orforglipron Metabolism

Orforglipron is metabolized primarily via hepatic cytochrome P450 3A4 (CYP3A4) to several oxidative metabolites. These oxidative metabolites are excreted into the intestinal lumen.2

Orforglipron Elimination

Orforglipron has 

  • an elimination half-life of approximately 29 to 49 hours after an oral dose, and 
  • a mean systemic clearance of 7.15 L/hour.2

After administration of a single oral dose of 2.5  mg orforglipron, 87% of the dose was recovered in feces (all metabolites) and less than 1% of the dose was recovered in urine.2
