What information does Lilly have for a patient who injects tirzepatide beyond the label?

There is no information on injecting tirzepatide IV or IM, or using injection sites other than the abdomen, thigh, or upper arm. There is no information on any differences of injecting between the abdomen, thigh, and/or upper arm.

Approved Injection Information

Labelling Information

Inject tirzepatide under the skin (subcutaneously) in the stomach (abdomen), thigh, or another person should inject in the back of the upper arm.1

Change (rotate) injection sites with each dose. You may use the same area of your body but be sure to choose a different injection site in that area.1

Safety and Efficacy Between Approved Injection Sites

Similar exposure was achieved with subcutaneous administration of tirzepatide in the abdomen, thigh, or upper arm.1,2

Eli Lilly and Company cannot comment on safety and efficacy differences when using one approved injection site versus another because this has not been studied.

Patients should consult with their health care providers if they have questions about which of the approved injection site(s) is best. Health care providers may consider the patient's individual factors and medical history.

Unapproved Injection Types or Locations

Injecting in Other Areas of the Body

Lilly has no clinical information regarding injecting the medication in other locations because there are no studies injecting in areas other than the abdomen, thigh, or upper arm.

Other Injection Types

Do not inject tirzepatide intramuscularly (IM) or intravenously (IV). The effect of IM or IV injection of tirzepatide has not been evaluated by Lilly; therefore, no data is available regarding this topic.7

  • Muscular tissues are typically more vascularized than SC tissues, which might accelerate drug absorption and influence dose response.3 It has been previously observed that IM injection of insulins led to higher fluctuations in glycemic level in patients with diabetes, and increased risk of hypoglycemia.4-6 The relevance of these results for tirzepatide is unknown.
  • After an IV injection, all the medicine is delivered directly into the blood stream.

Adequate needle length should be selected according to the specific population to be administered to reduce the risk of IM or IV injection.7
