Does Orforglipron Affect Appetite and Food Intake?

Orforglipron is a glucagon-like peptide-1 (GLP-1) receptor agonist that binds to and activates the human GLP-1 receptor.1

Glucagon-like peptide-1 is a physiological regulator of appetite and caloric intake. GLP-1 receptors are present in brain regions that regulate appetite. In animal studies, orforglipron distributed to and activated neurons in brain regions that regulate appetite and food intake.1

Orforglipron reduces body weight, with greater fat mass loss than lean mass loss. Orforglipron decreases food intake. This effect is likely mediated by decreased appetite.1

ATTAIN-1 and ATTAIN-2 were randomized, double-blind, placebo-controlled trials evaluating the efficacy and safety of orforglipron in adults with obesity or overweight. ATTAIN-1 enrolled participants without type 2 diabetes (T2D) and ATTAIN-2 enrolled participants with T2D.2,3

In ATTAIN-1 and ATTAIN-2, treatment-emergent adverse events of decreased appetite were reported in 5.8% to 8.4% and 8.2% to 15.3% of participants treated with orforglipron, respectively, compared with 3.2% and 2.9% of placebo-treated participants.2,3

These studies were conducted with an investigational orforglipron formulation at doses equivalent to the once-daily tablet dosages approved in the United States.1

Was Appetite Evaluated in Orforglipron Weight Management Trials?

In ATTAIN 1, the visual analog scale (VAS) was used to determine the effect of study drug on appetite and the desire for specific types of foods, while the Power of Food Scale (PFS) assessed cognitive and emotional responsiveness to food cues. Assessments were performed at week 0 (baseline) and weeks 24, 72, and 176, and post-baseline assessments were only conducted for participants with a baseline measurement.2

The appetite VAS was administered using a standardized pen and paper format consisting of eight questions evaluating

  • hunger
  • satiety
  • fullness
  • prospective food consumption, and
  • desire for sweet, salty, savory, and fatty foods.2

Each item was rated on a 100 mm scale, with each end representing the extreme end of the parameter (eg, not at all hungry, extremely hungry). Participants were to mark each respective line to indicate their rating.2

For the PFS, participants rated their agreement with a series of questions on a 5-point Likert scale, ranging from 1 (do not agree at all) to 5 (strongly agree) to measure the appetite for food based on 3 levels of food proximity, namely, food available, food present, and food tasted.2

Results from ATTAIN-1 on VAS and PFS have not been published.
