What is the recommended dose of OLUMIANT® (baricitinib) for pediatric patients with severe alopecia areata?

The recommended dosage of baricitinib for adults and pediatric patients 12 years of age and older with severe alopecia areata is 2 mg once daily orally, with or without food. Increase to 4 mg once daily if the response to treatment is not adequate.

What is the indication for baricitinib in adolescent patients with severe alopecia areata?

Baricitinib is indicated for the treatment of severe alopecia areata in adults and pediatric patients 12 years of age and older.1

The recommended dosage of baricitinib for adults and pediatric patients 12 years of age and older is 2 mg once daily orally, with or without food. Increase to 4 mg once daily if the response to treatment is not adequate.1

For adults and pediatric patients 12 years of age and older with nearly complete or complete scalp hair loss, with or without substantial eyelash or eyebrow hair loss, consider treating with 4 mg once daily, with or without food. Once patients achieve an adequate response to treatment with 4 mg, decrease the dosage to 2 mg once daily.1

Baricitinib is not recommended for use in combination with other Janus kinase (JAK) inhibitors, biologic immunomodulators, cyclosporine or other potent immunosuppressants.1

How is baricitinib administered in pediatric patients with alopecia areata?

Baricitinib tablets are given orally with or without food.1

Alternate Administration

For patients who are unable to swallow whole tablets, an alternative mode of administration, oral dispersion, may be considered.1

The 1-mg, 2-mg, or 4-mg baricitinib tablet(s), or any combination of tablets necessary to achieve the desired dose up to 4-mg, may be placed in a container with approximately 10 mL (5 mL minimum) of room temperature water, dispersed by gently swirling the tablet(s) and immediately taken orally.1

The container should be rinsed with an additional 10 mL (5 mL minimum) of room temperature water and the entire contents swallowed by the patient.1

What dose modification is recommended for baricitinib in patients with renal impairment?

The recommended dosage of baricitinib in patients with moderate renal impairment (estimated glomerular filtration rate between 30 and <60 mL/min/1.73 m²) should be reduced by half the recommended dose.1

What dose modification is recommended for baricitinib in patients with alopecia areata because of drug interactions?

Baricitinib exposure is increased when baricitinib is co-administered with strong organic anion transporter 3 inhibitors (such as probenecid), hence the dosage of baricitinib should be reduced by half the recommended dose.1
