Troubleshooting common Mounjaro® (tirzepatide) KwikPen® questions

Outlined below are steps to follow to troubleshoot common issues. This information should be used in conjunction with the Instructions for Use.

How to ensure that a full dose is delivered

Were the following steps performed?1

How to inspect the KwikPen

  • Ensure the pen is not damaged.1,2
  • The red inner seal should be flat and intact

  • Do not use the KwikPen if the red inner seal is damaged.1,2

Is there enough medicine in the KwikPen to deliver a dose?

  • The KwikPen contains 2.4ml (4 doses of 0.6mL) with an additional amount to allow the user to prime the pen. It is important users do not overprime their pens.
    • Steps 4 to 9 of the instructions of use outline these steps to prime their pens.
    • Prime position: Users should dial the dose knob until 2 clicks are heard and the extended line is shown in the dose window. This is the prime position.2
  •  The prime position.2
    • It can be corrected by turning the dose knob in either direction until the prime position lines up with the dose indicator.

  • Users may confirm that 4 doses have been delivered by looking at the plunger position.
  • Medicine will remain in the KwikPen after the 4th dose has been delivered. This is expected.
  • An additional 0.6mL (complete) dose cannot be dialled.
  • Discard the KwikPen and the remaining solution.1,2

Has the pen been primed?

  • The KwikPen has been successfully primed if a small amount of medicine comes out of the tip of the pen needle.
  • However, it is not necessary to see a stream of medicine in order for a successful prime.1,2

  • Users must follow the priming and dosing guidance in the Mounjaro KwikPen Instructions For Use, to ensure all 4 fixed doses can be delivered.

The dose knob is hard to push

To prevent the dose knob from being difficult to depress, consider the following to ensure that the KwikPen functions as it should;

How to prevent the dose knob from being difficult to push

Injection Technique:

  • Prime the KwikPen before each injection to remove air from the cartridge and to ensure the pen is working correctly.
  • If the dose knob is hard to press, push the dose knob down more slowly as this will make it easier to inject. 2

Proper Pen Needle Use: 

  • Do not apply pressure to the dose knob when the pen needle is not attached because there is no path for medicine to flow, which will increase the pressure in the cartridge and cause the septum to bulge and may result in medicine wastage when a needle is attached. 
  • Always attach a new compatible needle to the KwikPen before each dose. 
  • Ensure the needle is not damaged, bent or broken.
  • Confirm the pen needle is securely attached. This is essential in order for the medication to flow properly. 
  • The dose knob may be difficult to push if anything blocks or interrupts the flow of medicine into the pen needle.1

Proper Pen Storage:1,2

  • Store the KwikPen in a clean and dry place. The KwikPen
    • must NOT be stored with the needle attached, to prevent leaking, blockage of the needle and air entering the pen.
    • must be stored with the pen cap attached.
  • The dose knob may become hard to push if the pen is exposed to lint, dust, food or liquid.
    • If this occurs dispose of the pen. 

What steps can the user take if the dose knob is hard to push?

Select a new pen needle to attach to the KwikPen.2 

  • Patients should always use a new pen needle for each injection to help prevent infections and blocked needles.
  • Pull off the paper tab from the outer needle shield.


Push the capped needle straight onto the pen and twist the needle on until it is tight.2

Pull off the outer needle shield and keep it. This will be reused.2

Pull off the inner needle shield and throw it away.


Dial the dose knob back to zero "0".1

Press the KwikPen dose knob.2

Proceed with the priming steps below.

Priming steps 7 to 9

Step 7:2

  • Slowly turn the dose knob until 2 clicks are heard and the extended line is shown in the dose window. This is the prime position.
    • It can be corrected by turning the dose knob in either direction until the prime position lines up with the dose indicator.

Step 8:2

  • Hold the pen with the needle pointing up. Tap the cartridge holder gently to collect air bubbles at the top.

Step 9:2

  • Release some medicine into the air by pushing the dose knob in until it stops and then slowly count to 5 while holding the dose knob.
  • Check the dose window. You must see the "0".
  • Do not inject into the body

The priming was succesfull, if a small amount of medicine comes out of the tip of the pen needle.2 A stream of medicine coming out of the needle is not necessary, for the pen to be successfully primed.1
