What needles are recommended for the Mounjaro® (tirzepatide) KwikPen®?
Compatible needles, compliant with ISO 11608-2, must be used. No needles come in the pack with the medication.
Please note: A modified version of the Mounjaro KwikPen will be available on the market at the same time as the initial version. While the two versions may look slightly different, they work in the same way.1 The information provided in this document applies to both the modified and initial KwikPens.
What needles are compatible with the KwikPen?
The Tirzepatide KwikPen is compatible with ISO 11608-2 compliant pen needles from brands such as Embecta (formerly BD), Novo Nordisk, and Terumo, in any length and gauge combination:
- Lengths: 4 mm, 5 mm, 6 mm, 8 mm, 12.7 mm
- Gauges: 29G, 30G, 31G, 32G, 33G, 34G.1
Not all length and gauge combinations are commercially available. Recommendations for the type of needle and proper injection technique remain at the HCP's discretion.1

What needle length should be used?
Mounjaro supplied in KwikPen should be administered via SC injection.1 Pen needles available for SC injection typically range from a length of 4 mm to 12.7 mm (5/32 inch to 1/2 inch).2
According to a review of the literature, pending availability of needle length, shorter needles are more appropriate and generally better tolerated. Pen needles of 4 mm are suitable for adult patients.3,4
The length of the needle does not affect how much Mounjaro solution is injected.1
The decision on needle gauge and length remains at the discretion of the physician or pharmacist.5
Why are there no needles in the pack?
Eli Lilly and Company does not manufacture any needles.
Furthermore, the Mounjaro KwikPen is compatible with different kind of needles, including needles of different sizes, and needles of different manufacturers with different characteristics. Supplying medicine and needles separately allows the health care professional and patient to choose a needle that is appropriate for the patient.
